Welcome to the new home of Fairfield Community Health Center. We are in the process of building and updating our webpage, we apologize if sections are lacking or incomplete.

Fairfield Community Health Center is a Federally Qualified Health Center or FQHC. FQHC’s are community-based and patient-directed organizations that serve everyone, especially populations with limited access to health care.

Fairfield Community Health Center is open Monday thru Thursday from 7:30 am to 4:30 pm and Friday from 8 am to 3 pm. The Health Center is closed everyday from 11:45 - 12:45.

Fairfield Community Health Center accepts Medicaid, Medicare, CareSource and Molina. The Health Center also accepts self-pay on a sliding fee scale which is based on proof of income and family size.
