Fairfield County Health Needs Assessment |

This health assessment provides us with a snapshot of Fairfield County, as well as our state and nation. This snapshot is only a beginning. The data presented in this report provide valuable information we will use to develop strategies that focus on wellness, access to care, and unmet community health needs. The assessment also gives us additional insights in the areas of health and well-being and how they relate to our community structure as it is today. All three participating agencies, the Fairfield Department of Health, Fairfield Medical Center and the Fairfield Community Health Center came together to help each other meet this critical unmet need. All three organizations are regularly required to assess the health care status of their community, using a nationally accepted and standardized format, as the basis to write better grants, formulate strong strategic plans, and answer calls to action.
